Keeping a medication record for your pet is not about being overly cautious. It is about having accurate information available at the moment you need it most — which is rarely a planned moment.

What to Record for Each Medication

Medication Name

Use the full name, not just "the pink tablet" or "the ear drops." If your pet is seen by an emergency vet, they will need to know exactly what has been prescribed.

Dose

Exact dose in milligrams or millilitres, and the weight your dose was calculated for. If your pet's weight changes, this matters.

Frequency and Timing

Once daily? Twice daily? With food or without? Specific times? Log what was prescribed and what you are actually doing — these sometimes differ.

Start and End Dates

Especially important for antibiotics and short-term medications. An incomplete course of antibiotics can have real consequences.

Prescribing Vet

Which practice? Which vet? This is useful if you have a query or need a repeat prescription.

Reason for Prescription

"After surgery" or "bacterial skin infection, September 2025" is much more useful than a blank field when you are trying to remember why your dog was on something eighteen months ago.

Side Effects or Observations

Did the medication cause lethargy, appetite changes, or digestive upset? Noting this helps your vet make informed decisions about future prescriptions.

Allergies and Adverse Reactions

If your pet has ever reacted badly to a medication, this must be recorded and communicated clearly at every vet visit.

Ongoing Medications

For pets on long-term or preventative medication — flea and tick prevention, heartworm, joint supplements, or thyroid medication — a running log with the date each dose was given prevents accidental double-dosing and makes it easy to confirm compliance at vet appointments.
